Welcome all to ESDH - Below are two forms that we are requesting each student's parent/guardian to fill out and submit through the links provided.  We do require these forms to be completed every school year.

Chromebook Loan Form
ESDH will be able to offer a Chromebook loan for the 2023-2024 school to all students, who do not have a device they can bring to school.  Students will be expected to have the device with them for every class.

Permission Slip
This permission form allows students to go off property as part of a class activity with teacher supervision during class time with the required ratio of 15 students to 1 teacher/staff member ratio.  This allows for teachers to incorporate more outside activities into their classes.   (Please note - students are able to off property during lunch time**no permission slip is needed)
